合併CKD的心衰患者植入ICD有害無益？
慢性腎臟疾病（CKD）[1]是一個嚴重的公共健康問題，在美國約14%的成年人存在CKD[2]。心血管疾病是CKD患者發病和死亡的主要原因[3]，而心力衰竭是最常見的心血管疾病之一[4-6]。同時，估計美國有超過570萬成人患有心力衰竭，其中的30％同時合併CKD[7,8]。心力衰竭的主要併發症之一是心臟性猝死，而CKD是導致心臟性猝死的最重要危險因素[9]。
在隨機對照研究中，與單獨使用最佳藥物治療相比，植入式心臟復律除顫器（ICD）是心臟性猝死的主要預防策略，顯著降低左心室射血分數（LVEF）下降心力衰竭患者心律失常死亡風險[10-12]。然而，在現有研究中有關ICD在合併CKD的心衰患者中的研究證據不足。由於ICD的植入有一定併發症風險且價格昂貴，評估心衰合併CKD患者植入ICD的有效性和安全性十分重要。
現有的合併CKD的心衰患者植入ICD進行一級預防研究較少，或受到樣本量的限制[13-15]，或研究對象代表性不強[9，16]（與目標心力衰竭患者相比，其代表性更差[17]），且缺乏沒有接受ICD植入的心衰合併CKD患者作為對照組[13,18]。因此，剛剛發表在JAMA Internal Medicine上的一項非干預性觀察研究[19]，旨在比較合併CKD的心力衰竭患者植入ICD與死亡和住院風險的關係。
研究入選流程見圖1。患者均為射血分數≤40%，且腎小球濾過率低於60ml/min/1.73m2，接受ICD植入，與未接受ICD植入的患者按照CKD階段、年齡等進行1:3的匹配。共5877例射血分數下降同時合併CKD的心衰患者入選，其中ICD植入組1556人，未植入ICD組4321人。ICD組患者LVEF低於未植入組（26.% Vs 29.4%），且具有明確冠心病病史者比例較高（37.1% Vs 30.1% ），服用泮利尿劑比例更高（82.8% Vs 73.5%）。具體見圖2。

平均隨訪3.1年後，共有2541（43.2%）例患者死亡。ICD組每100人年的死亡率為14.9（95％可信區間：13.9-16.1/100人年），而未植入ICD組每100人年為13.6（95％可信區間：13.0-14.2 / 100人年），未見明顯統計學差異。此外，ICD植入和全因死亡之間的關聯無統計學差異，具體結果見圖3及圖5A。
隨訪期間共有1922例患者因心衰再次入院，ICD組的心衰再入院率明顯高於未植入ICD組（16.9/ 100人年vs 11.2/ 100人年）。在Poisson模型中，ICD植入與心衰相關再入院具有相關性，即使在校正人口學、合併症和藥物使用後，這種相關性仍然存在，具體結果見圖4和圖5B。對於全因住院率分析後，同樣ICD組患者明顯高於無ICD植入組患者，具體結果見圖4和圖5C。

該研究結果提示：對於心力衰竭合併CKD患者，ICD植入不能改善心衰患者生存率，且增加心力衰竭患者全因住院風險。因此心力衰竭合併CKD患者應更加慎重考慮ICD植入的潛在風險和獲益。
