wound care

1

The skin isthe largest organ of the body, making up 16% of body weight.

It has several vital functions, which include; immune function, temperature regulation, sensation and vitamin production.

Skin is a dynamic organ ina constant state of change;cells of the outer layers continuously shed(脫落)and are replaced by inner cells moving to the surface.

2

structure

Epidermis/,ep?"d??m?s/

Dermis/"d??m?s/

Hypodermis/subcutaneous layer /s?bkju?"te?n??s/

Epidermis: Is the outer layer of the skin.

comprised of epithelial cells

avascular

0.04mm thick

regenerated every 2-4weeks, subject to an individual"s age and friction forces applied to the skin

receives nutrients from the dermis below

comprised of 4 to 5 layers depending on the body location

Dermis: Is the middle layer of the skin and is approximately 0.5mm thick subject to anatomical site

made up of two layers

is very vascular

contains nerves, connective tissue, collagen, elastin and specialized cells such as fibroblasts and mast cells

responsible for inflammatory reactions which occur in response to trauma and infection

receptors for heat, cold, pain, pressure, itch and tickle

Hypodermis: Is the inner most layer of the skin and is referred to as the subcutaneous layer

supports the dermis and epidermis

varies in thickness and depth

comprised of adipose tissue, connective tissue and blood vessels

the function is to store lipids, protect underlying organs, provide insulation and regulate temperature

Skin Appendages/?"pendid?iz/:

Includes Sweat glands, hair, nails and sebaceous glands which are all considered epidermal appendages.

how

to care

Sophie: Hi

Ali: Hi. Sophie. Thanks for coming up to the ward. it"s about Mr Jones in bed five.

Would you mind giving me some advice on his wound

care management?

Sophie: Yeah, sure. Can you fill me in on his past history first?

Ali: Um, right. Mr Jones is a 68-year-old smoker with a long history of PVD(玻璃體後脫離).

He developed a venous ulcer on his right ankle after he tripped on some stairs at the back of his house. That was, er, about six months ago

His local doctor had a look at it and asked the

District Nurses to come and dress the wound

at home. Um,they"ve been dressing the wound

three times a week.

At that stage it was hoped

that he could avoid coming into hospital. He

lives with his wife who needs quite a lot of help

as she has chronic asthma. It puts quite a lot of

strain on her if he has to be hospitalised

Sophie: Right, that"s difficult, isn"t it?

Obviously the ulcer didn"t improve at home, so

what happened next?

Ali: Um. Two weeks ago he was admitted

to this ward to have an assessment of

his circulation and to monitor his wound

management. Hehad a Doppler test done

er, last week whichindicated poor blood

circulationin his lower legs. Um, he"s also been

spiking temperatures(體溫飆升) and the surrounding skin

of the wound has become quite reddened. We

sent a wound swab off, and we just got the

results yesterday. Of course, he "s growing a few

bugs(蟲子) so he"s started on some IV antibiotics.

we"re unsure of the best wound management

for him. What would you recommend that we

change to?

Sophie: Hm, let me have a look at the wound

and well see what the best option is.
